Please fill in the sample submission form below.

If we need further information regarding the experimental setup of your project, you will be contacted by a representative from the Microarray center.

Please call Susanne Smed or Elisabeth Schiefloe at phone number: +45 3545 4208 in order to arrange delivery of the samples to the Center.

It is requested that you make a print of this page after submission and add the date and your signature at the bottom. This signed document should be included with the samples.
